About the Role
Provide comprehensive social work services for AltaMed PACE frail elderly participants, coordinating community resources, conducting psychosocial assessments, and participating in interdisciplinary team care planning.
Requirements
MSW with California LCSW or ACSW registration; geriatric experience preferred
Bilingual Spanish/English required for East LA and Montebello PACE participant population
Experience with Medicare/Medi-Cal PACE dual-eligible benefit structure
PACE IDT participation, care planning, and social needs documentation experience
