About the Role
Deliver hybrid inpatient and community-based palliative care to AltaMed's seriously ill patient population, focusing on goals-of-care conversations, symptom management, and care coordination with PACE and CCM teams.
Requirements
NP with California license and national board certification (FNP, AGNP, or ACHPN)
Palliative care certification or specialized training strongly preferred
NHSC loan repayment eligible; bilingual Spanish/English a strong plus
Experience with POLST, advance care planning, and serious illness communication
