About the Role
Conduct home-based primary care visits for AltaMed's homebound and PACE participant population across Los Angeles, managing chronic conditions, medication reconciliation, and coordinating with IDT for complex care needs.
Requirements
NP with California license; FNP or AGNP board certification preferred
Valid California driver's license; reliable transportation for home visits across LA County
Experience with home health, PACE, or Complex Care Management programs
Bilingual Spanish/English strongly preferred; OCHIN Epic proficiency a plus
