About the Role
Conduct occupational therapy assessments and provide evidence-based interventions for AltaMed PACE participants, supporting ADL independence, home safety evaluations, and adaptive equipment needs for frail elderly members.
Requirements
OTR/L with California OT license required; geriatric or PACE experience strongly preferred
Experience with ADL assessments, home environment evaluations, and fall prevention programming
PACE IDT participation and care planning documentation experience preferred
BLS/CPR required; Spanish language skills a plus for participant communication
