About the Role
Provide family medicine and geriatric primary care for AltaMed's aging patient population, managing complex chronic conditions, supporting PACE program participants, and conducting comprehensive geriatric assessments.
Requirements
MD/DO with California license; board certification in Family Medicine or Internal Medicine with geriatric training/CAQ
FQHC experience preferred; comfort managing frail elderly and PACE populations
NHSC loan repayment eligible; bilingual Spanish/English a strong asset
Experience with comprehensive geriatric assessment, dementia care, and polypharmacy management
