About the Role
Care coordinator at Beverly Community Clinic navigating patients to medical, behavioral health, and social services across 3 LA sites. Serving predominantly Latino, uninsured, and immigrant populations in the Pico-Union/West Adams area.
Requirements
Bachelor's degree preferred or equivalent experience
Bilingual Spanish/English required
Social services navigation experience in LA County
EHR or case management software experience
Cultural competency with immigrant and Latino communities
