Requirements
BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
Spanish language skills preferred
OCHIN Epic documentation experience preferred
Valid CA driver's license for community-based care visits
Cultural humility with diverse San Gabriel Valley immigrant communities
