About the Role
ECM Care Coordinator at CMC's Modesto site — coordinating complex Medi-Cal patients in the Central Valley farmworker community. OCHIN Epic, home visits, and CalAIM community supports navigation.
Requirements
BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
Spanish fluency required
OCHIN Epic documentation experience preferred
Valid CA driver's license for field-based ECM visits
Knowledge of San Joaquin and Stanislaus County social services
