About the Role
ECM Care Coordinator at CommuniCare's West Sacramento clinic. Bilingual Spanish/English ECM coordination for complex Medi-Cal patients — home visits, OCHIN Epic documentation, social needs navigation.
Requirements
BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
Spanish fluency required
OCHIN Epic documentation experience preferred
Valid CA driver's license for community-based ECM visits
Knowledge of Yolo County social services, housing resources, and Medi-Cal
