About the Role
Coordinate care for Clinica Sierra Vista's high-risk Medi-Cal members enrolled in ECM, conducting outreach, home visits, and care plan coordination for Kern County's agricultural and farmworker communities.
Requirements
CHW certification or BA/BS in social work, public health, or related field
Bilingual Spanish/English required; experience working with farmworker or agricultural communities a plus
CalAIM ECM documentation knowledge; Medi-Cal managed care navigation experience preferred
Valid California driver's license for home visits across Kern County service area
