About the Role
Provide mobile, team-based case management for unhoused individuals in San Diego under FHCSD's Street Medicine program, coordinating shelter, healthcare, and social services for high-acuity homeless patients.
Requirements
Bachelor's in social work, nursing, or related field
Experience with unhoused populations, harm reduction, and trauma-informed care
CalAIM ECM or PATH/HOPWA case management experience a strong plus
Valid CA driver's license required for field-based work; bilingual Spanish preferred
