About the Role
Supervise FHCSD's Enhanced Care Management team, overseeing caseload management for high-utilization Medi-Cal members, ensuring CalAIM encounter documentation, and coaching care managers on complex case navigation.
Requirements
BA/BS in social work, public health, or healthcare; LCSW, RN, or equivalent licensure preferred
3+ years ECM, CCM, or care coordination experience in FQHC or Medi-Cal managed care setting
Strong knowledge of CalAIM ECM program requirements, documentation standards, and encounter billing
