About the Role
ECM Care Coordinator at IFMG in San Bernardino County. Inland Empire's complex Medi-Cal patients — farmworker families, low-income suburban households, and recently arrived immigrants.
Requirements
BA/BS in social work, health education, or 3+ years ECM/CCM experience
Spanish fluency required
Cerner EHR experience preferred
Valid CA driver's license for community-based visits in SB County
Knowledge of San Bernardino County social services, housing resources, and Medi-Cal
