About the Role
Deliver integrated behavioral health services embedded in La Clinica's primary care teams across East Bay sites, providing brief interventions, warm hand-offs, PHQ-9 screenings, and care coordination for patients with mental health and substance use needs.
Requirements
Master's degree in social work, counseling, or psychology required
LCSW, LMFT, LPCC, or associate licensure (ASW, AMFT, APCC) required
Integrated primary care or co-located behavioral health experience preferred
Bilingual Spanish/English strongly preferred; OCHIN Epic experience a plus
