About the Role
Connect La Clinica patients with social services, benefits enrollment, housing navigation, and community resources, providing case support and warm referrals across East Bay clinic sites under CalAIM Community Supports.
Requirements
High school diploma required; bachelor's degree in social work or public health preferred
Bilingual Spanish/English required — bilingual proficiency assessed during hiring
Experience with benefits navigation, social services referrals, or CHW role in community health
Knowledge of Medi-Cal Community Supports, ECM, and local Alameda/Contra Costa social service resources
