About the Role
ECM Care Coordinator at Livingston Community Health in Merced County. Complex Medi-Cal patients — farmworker families, Sikh agricultural workers, and immigrant communities in the Central Valley.
Requirements
BA/BS in social work, health education, or 3+ years ECM/CCM experience
Spanish fluency required; Punjabi a plus
NextGen EHR experience preferred
Valid CA driver's license for community-based visits in Merced County
Farmworker or agricultural community experience preferred
