CHW at Mission Area Health Associates connecting Mission District community members to health, housing, and social services across 5 SF sites. Outreach to Latino, undocumented, and immigrant communities navigating Medi-Cal, SF DPH, and community resource networks.
Requirements
Bilingual Spanish/English required
Mission District community ties strongly preferred
Medi-Cal and SF DPH program knowledge
Undocumented patient access and benefit navigation experience
