About the Role
Navigate MNHC's HIV-positive patients through care coordination, medication adherence support, Ryan White Part A services, and social determinants screenings in the heart of SF's Mission District.
Requirements
High school diploma or equivalent; some college or health certification preferred
Bilingual Spanish/English required; experience working with HIV+ populations or LGBTQ+ communities
Knowledge of Ryan White Part A services, ADAP, and SF DPH HIV resources
Experience with Medi-Cal, SSI/SSDI benefit navigation, and housing resources preferred
