About the Role
CHW at The Roads Foundation connecting Compton and South LA residents to health care, housing, and social services across 9 sites. Conduct community outreach, SDOH screenings, and patient navigation for a predominantly Black and Latino Medi-Cal population.
Requirements
CA CHW certification (SB 184) preferred
Bilingual Spanish/English required
Compton or South LA community ties preferred
LA County social services and Medi-Cal navigation
Motivational interviewing and trauma-informed outreach
