ECM Care Coordinator at RUHS-CHC's Indio site serving Eastern Coachella Valley — farmworker families, indigenous communities, and resort/service worker households. Epic EHR, CalAIM community supports navigation.
Requirements
BA/BS in social work, health education, or 3+ years ECM/CCM experience
Spanish fluency required
Epic EHR experience preferred
Valid CA driver's license for community-based visits in the Coachella Valley
Knowledge of Riverside County social services, Medi-Cal, and CalAIM pathways
