Requirements
BA/BS in social work, health education, or 3+ years ECM/CCM experience
Spanish fluency required
eClinicalWorks EHR experience preferred
Valid CA driver's license for South Bay community-based visits
Knowledge of LA County South Bay social services and Medi-Cal
