CHW at Santa Clara Valley Health Centers connecting South Bay's diverse immigrant communities to ECM and community supports. San Jose is one of CA's most linguistically diverse cities — Vietnamese, Spanish, and Tagalog CHWs especially needed.
Requirements
CHW certification or 2+ years community health outreach
Multilingual skills (Spanish, Vietnamese, or Tagalog) highly valued
Knowledge of Santa Clara County social services, Medi-Cal, and housing resources
Valid CA driver's license
South Bay immigrant community lived experience preferred
