About the Role
Supervise San Ysidro Health's Enhanced Care Management team serving 3,300+ staff organization, overseeing caseload quality, CalAIM encounter documentation, and team performance for one of California's largest border FQHCs.
Requirements
BA/BS with LCSW, RN, or equivalent licensure; 3+ years ECM or complex care management experience
CalAIM ECM program requirements, DHCS compliance, and managed care plan coordination expertise
Bilingual Spanish/English preferred; supervisory experience with multidisciplinary care teams
Epic care management module experience; experience at large FQHC or managed care organization
