About the Role
Provide RN care management for San Ysidro Health PACE participants, conducting health assessments, developing IDT care plans, and managing acute illness episodes for frail elderly border community members.
Requirements
RN with California license; geriatric, home health, or PACE experience strongly preferred
Bilingual Spanish/English preferred for predominantly Spanish-speaking PACE participants
PACE IDT experience, care planning, and Medicare/Medi-Cal PACE documentation knowledge
BLS required; valid driver's license for home visit coverage preferred
