Requirements
BA/BS in social work, health education, or 3+ years ECM/complex care coordination
Spanish fluency required
Valid CA driver's license for community-based ECM visits
Cerner EHR experience preferred; training provided
Knowledge of Ventura County social services, housing, and Medi-Cal
