How this role generates billable revenue

• Care-management and navigation services may be covered by Medicare, Medicaid, or a payer contract; verify the applicable program and the patient's eligibility
• CHWs and other auxiliary personnel may support qualifying CHI or PIN work when patient, initiating-practitioner, supervision, consent, documentation, code, and payer rules are met; the FQHC or authorized practitioner submits the claim
• Standardized social-needs assessment and closed-loop referrals may support covered care when the selected service's requirements are met
• Manage an appropriate care-management panel and document enrollment, follow-up, and retention outcomes that support access and compliant reimbursement.
• Use your bilingual fluency and community trust to reach patients providers can't — that's the access the mission is built on.

• CalAIM Enhanced Care Management (ECM) — Billable, intensive care management for high-need Medi-Cal members — staffed by lead care managers, CHWs, RNs, and BH staff (~per-member-per-month).
• CalAIM Community Supports — Billable in-lieu-of services (housing navigation, medically tailored meals, recuperative care) that care coordinators and CHWs deliver.
• CHW Medi-Cal benefit + Dyadic & Doula benefits — CHW services, dyadic (parent-child) behavioral services, and doula care are all Medi-Cal-billable maternal/community revenue lines.
