How this role generates billable revenue

• Nurse-supported chronic-disease workflows and team visits may contribute to qualifying practitioner services when scope, supervision, documentation, code, and payer rules are met
• Chronic Care Management (99487/99490/99491), Transitional Care Management (99495/99496), and Remote Patient Monitoring may be reportable by the FQHC for eligible patients when all requirements are met
• Support diabetes and hypertension panels and team visits within nursing scope so the care team can expand access; reimbursement depends on the qualifying practitioner service and payer rules.
• Support qualifying TCM after discharge and remote monitoring between visits under the required practitioner oversight and documentation workflow.
