About the Role
CHW connecting rural NorCal residents to Ampla Health's primary care and social services. Field-based outreach in Butte and Glenn Counties — including agricultural communities, unhoused residents, and disaster-displaced populations (post-Camp Fire).
Requirements
CHW certification or 2+ years community outreach experience
Valid CA driver's license — extensive rural travel required
Spanish language skills helpful for agricultural worker outreach
Experience with unhoused populations or disaster-affected communities a plus
Knowledge of NorCal social services and Medi-Cal enrollment
