About the Role
RN role on BACH's primary care team in Fremont. Panel management, chronic disease coaching, care coordination for a diverse East Bay community including Filipino, Afghan, and Latino families.
Requirements
Active California RN license required
1+ years clinical nursing experience; FQHC or community health preferred
Multilingual skills (Tagalog, Spanish, or Cantonese) highly preferred
OCHIN Epic experience a plus
Cultural competency serving diverse immigrant communities required
