About the Role
ECM Care Coordinator at BACH's Newark clinic. Manage high-risk Medi-Cal patients through CalAIM Enhanced Care Management — home visits, warm handoffs, social needs navigation in Fremont/Newark/Union City.
Requirements
BA/BS in social work, health education, or related field; or 3+ years ECM/care coordination
Spanish or Tagalog fluency preferred
Valid CA driver's license for community-based ECM visits
OCHIN Epic documentation experience preferred
Culturally responsive care for Filipino, Afghan, and Latino patients required
