ECM Care Manager at CARES Community Health, a 7-site Sacramento FQHC. Provide intensive community-based care management for high-complexity Medi-Cal members — connecting patients to housing, BH, and social services through CalAIM Enhanced Care Management.
Requirements
RN, LCSW, or bachelor's in social work/nursing with 2+ years care management experience
ECM or complex care management experience preferred
CalAIM and Medi-Cal program knowledge
Community-based, field-facing care management skills
Social determinants of health documentation and navigation
