Requirements
MD/DO with California medical license; Board Certified in Family Medicine or Internal Medicine
NHSC Loan Repayment eligible — Contra Costa County HPSA designation
Epic EHR enterprise-wide proficiency preferred
Spanish language skills preferred — significant West Contra Costa County Latino population
Interest in complex care, HCH, or PACE programs a plus
