Community Health Representative (CHR) at Indian Health Center of Santa Clara Valley, serving AI/AN community members across 13 Santa Clara County sites. Conduct wellness outreach, patient navigation, and cultural support in alignment with tribal traditions and Indian Health Service CHR model.
Requirements
American Indian or Alaska Native identity strongly preferred
Community health, social services, or outreach experience
Knowledge of tribal health concepts and Indian Health Service programs
Motivational interviewing and health coaching skills
