About the Role
Deliver adult primary care to La Clinica's East Bay patient panels, focusing on chronic disease management for complex Medi-Cal patients including ECM-enrolled adults with multiple comorbidities.
Requirements
MD or DO with current California license; Board Certified or Eligible in Internal Medicine
Experience managing chronic disease panels in an FQHC or community health setting
Knowledge of Medi-Cal managed care, PPS billing, and CalAIM preferred
Bilingual Spanish/English preferred; OCHIN Epic familiarity a plus
