About the Role
CHW at Coppertower Family Medical Center connecting Sonoma County agricultural workers and rural families to health and social services. Conduct outreach, patient navigation, and SDOH screenings across 3 sites in the northern Sonoma corridor.
Requirements
CA CHW certification (SB 184) preferred
Bilingual Spanish/English required
Farmworker or agricultural community outreach experience
Knowledge of Sonoma County social services
Valid California driver's license for community visits
